Medical Form

All participants are required to complete the following information which will be treated as strictly confidential. The challenge in which you will be participating will require a good level of fitness, strength and endurance. If you have any doubts about your health, you should check with your doctor to ensure that you are sufficiently fit, healthy and mobile to participate in the challenge.

There will be trained medical personnel, with first aid supplies, with the group at all times. They will administer first aid if and when required. Please note, routes may be away from main cities and hospitals. If you have a pre-existing medical condition, have answered yes to any of the questions below and / or are over 65, we may need to contact you for further information and will require a form signed by your doctor. Likewise if you develop any new medical conditions or experience a worsening of an existing condition after completing the below, you must inform Norwood.
